Who LCCR Helps
My clients are those that have either stepped into caregiving for a loved one or have unexpectedly thrown into that role and are seeking an organized plan that fits their loved ones needs, support, and trusted guidance.
What LCCR Provides
Overall assessment of the loved ones needs and the family's needs.
Create a tailored care plan and organization forms for everyone to easily follow that fits the loved one’s care and the family
Create tailored check lists to easily track all aspects of care. This will be vital information for the doctors and providers and enable them to provide the best care possible.
Assist in communicating with the providers, hospice and family. Sometimes we don't know the questions that need to be asked or understand the answers we are given. This is where my experience will help interpret that information.
Weekly check-ins and oversight to ensure the plan is efficient and adjust quickly when new care needs arise
New caregivers- I will help you hire and on board a care team or work with the care team you have, teach routines that best fit your loved one, house rules and tailored care instructions. I will ensure they follow the simple but critical check lists. We will utilize your care team to help lift the burdens.
Working with the family - I will work closely with your family, sharing insights and options so that you can make the best informed decisions.
Transition guidance when needs change. Hospital discharge, facility placement, moving loved ones, respite care. I will help you with a plan or connect you with trusted resources.
Important Note: We are NOT a home care agency. I provide consulting and care coordination. Caregivers are hired by you and paid directly by you.
FEES
During this 30 minute call, we assess your care situation: including your loved ones needs, your family situation and needs, what plan you already have or if you don't have one yet, and then decide if my services can help.
Phone or Video - $ 75/hour.
5 hour on site minimum for the first visit and a 3 hour on site minimum after first visit.
Organization of daily care, training for family and care team, tailored check lists, communication with providers, Hospice and staff. Assistance with new changes in care, with the care team, updating tailored check lists,and staying proactive and organized.
$2,500.00/month
Inclusive Care Management Plan:
*additional fees for traveling outside the Austin Area
This Inclusive Care Management Plan has been called “Come in and take the bull by the horns and run the show” by former clients.
In person 5 hour assessment, detailed organization of client care, hands on training for family and care team, tailored check lists, communication liaison with family, providers, Hospice and care team.
Help interview and help hire private caregivers and/or work with agency caregivers of your choice if needed.
Utilize the caregiving team to allow them to be more efficient during their shifts.
Organize a light chore list and delegate small tasks for your care team.
Assess equipment and products that could be helpful or are needed for the loved ones specific care. (you may not know what is available to you or what you might need)
Weekly 2 hour onsite check-in and oversight, always staying proactive rather than reactive. Weekly assessment and tracking patterns in any changes in care so we can immediately make changes that are needed.Adjusting care needs as they arise, sooner than later.
Checking in with the care team and giving guidance where needed.
Keeping the family updated and informed.
Organizing and training for overnight care and 24 hour care.
Guidance with scheduling so you have efficient schedules that work best for the client and the family.
Recommending future care plans and preparing the care team so that we ensure that we are looking forward and be better prepared for what is coming. Keeping everyone on the same page and informed on what to expect.
Connecting you with trusted resources for other care needs if needed. Such as Legal services, financial services, transportation, respite care, end of life planning, downsizing, finding a placement facility, etc.

Maybe this fits your situation because your situation needs someone reliable to come in, assess the situation and make a tailored care plan. Organize overall care, recommend a care team and/or train the family. Set everything up and then continue to oversee, manage care, stay organized and make necessary changes whenever your loved ones care changes, all while consulting with you and your family and keeping you well informed. This is a great fit for a situation when a family just does not have the time or availability to give full time care.
